Referral Process

The Referral Process

The referral process begins with an enquiry, by telephone, email or letter. The service will then ask for a referral form to be completed giving brief information about the potential service user and their support needs. A professional from the multi-disciplinary team can complete this form.

A provisional review of this information is then undertaken to assess whether the service can potentially meet the needs of the individual being referred. If the service feels they can meet the person’s needs, a full assessment will then be arranged with the potential tenant and their care team.



Referral Criteria

Our service is designed for individuals who may be transitioning from secure hospitals, inpatient settings or community placements and require ongoing nursing oversight and specialist support within a small residential environment.  



we welcome referrals for adults ages 18-65 years who present with : 

  • Mental health conditions
  • learning disabilities
  • Autism spectrum disorder 
  •  Personality Disorders
  • Dual Diagnosis 
  • complex behavioural needs 
  • individuals who require structured and consistent support 


Our service is particularly suitable for individuals who would benefit from smaller environments with consistent staffing and specialist clinical oversight

The Assessment Process


At this assessment, the following aspects of the service will be discussed with the service users and their care team:

  • Current support needs
  • Cultural needs
  • Rehabilitation needs
  • Social integration needs 
  • Identified areas of risk.
  • What things are important to them as an individual?  
  • What they are seeking from the service.
  • Proposed plan of care to be offered by the service.equirements.

Funding

We accept placements funded through :

  • Local Authority funding 
  • NHS continuing healthcare 
  • joint funding arrangements 
  • section 117 aftercare
